How do we ensure data quality in health facilities with high staff turnover?
Data quality is a persistent challenge in our facility, partly because we have high staff turnover and new staff are often not adequately trained on data collection and reporting. What strategies have worked for others in maintaining data quality despite staff changes?

Standard operating procedures (SOPs) for data collection and entry are essential. When SOPs are well-documented and accessible, new staff can follow them without needing extensive training. We laminated our SOPs and posted them at each data entry point.
Regular data quality audits — even simple ones done monthly — help identify problems early. We do a quick check of completeness and consistency at the end of each month and feed back the results to the data entry staff. This creates accountability and helps staff understand why data quality matters.
