A comprehensive health facility assessment typically covers: infrastructure (buildings, water, sanitation, electricity); equipment and supplies (medical equipment, medicines, diagnostics); human resources (staffing levels, qualifications, training); service delivery (availability and quality of services); health information systems (data collection, reporting, use); and governance (management, supervision, accountability). The WHO Service Availability and Readiness Assessment (SARA) tool is widely used for health facility assessments and has been adapted for use in South Sudan.
The DHIS2 Health Facility Registry can be used to maintain an up-to-date database of health facilities and their key characteristics. Linking facility assessment data to the registry enables tracking of changes over time and comparison across facilities.
