Best practices for monthly data reconciliation?

Colleagues, how do you reconcile facility paper registers with the monthly HMIS report? Our county has recurring discrepancies in ANC and immunisation counts. I will share what worked for us after hearing your approaches.

Great question. In our state we introduced a daily tally sheet that is checked by the facility in-charge every Friday — discrepancies dropped noticeably.

We do something similar. The key is making the check routine, not an audit event.
